Plan Benefits
For an annual fee, valid for one year from the date of enrollment, members receive:
- Two routine exams and cleanings within a 12-month period.
- All necessary x-rays at no additional cost.
- Two fluoride treatments at no additional cost.
- 15% discount on diagnostic, preventative, periodontal, and restorative treatments that do not require lab work.
- 10% discount on root canals, crowns, bridges, and dentures.
Important Considerations
- Some procedures may require a referral to an outside specialist, such as orthodontic care or wisdom tooth extractions. These treatments are not included in the plan.
- Payments for services under this plan must be made by check, cash, or debit card. Credit card payments are not accepted.
- This is not an insurance plan, meaning there are:
- No deductibles.
- No annual maximums.
- No waiting periods.
- No exclusions for pre-existing conditions.
- No claim forms.
- No exclusions for cosmetic treatments.
